Issues involved in the management of Gastro-intestinal cancers

The G.I. Surgery (Surgical Gastroenterology) is now well established and nearly ten major institutions in our country are giving M.Ch in G.I. Surgery as per the norms of Medical Council of India. Lately, further offshoots of this superspeciality, i.e. hepato-pancreato-biliary group (HPB) and colorectal group are also receiving due recognition by various organizations and this is resulting in better patient outcome. However on the other side generalist oncosurgeons are performing relatively few anecdotal G.I. Oncological cases. It is high time that G.I. Oncology should be treated as a separate entity within the Surgical Gastroenterology.

Important :- Each cancer patient will be subjected to the “Tumour Board” consisting of G.I. Oncologist, Medical Oncologist & Radiation Oncologist. The Treatment protocol of each patient will be decided as per International norm and will be discussed with patient and family members.

The best therapeutic strategy for a given patient will be decided and subjected to surgery followed by + chemo and radiotherapy OR first Neoadjuvent Chemo - Radiotherapy followed by surgery depending upon the concensus of the Tumour Board.
